Stephanie Staveland, LCSW and Breanna Kilmartin, LCSW are considered "IN NETWORK" with Premera Blue Cross Blue Shield and Aetna. Stephanie is also in-network with Cigna. For all other insurance plans she is able to bill OUT OF NETWORK.
Rebekah Bailey, MSW is able to see self-pay clients and is able to supervisory bill Aetna. Rates are provided below.
We accept full fee at or before the time of session via cash, debit, credit or FSA/HSA card. Clients are required to keep a working credit card on file and will be charged their estimated copay at the time of service.
We are unable to accept Medicaid/Denali Kid Care/Medicare at this time.
We understand that life happens and there will be times you cannot make it to a scheduled session. We kindly ask that cancellations are made 24 hours in advance of your scheduled session or a cancellation fee of $75.00 will be charged to your account. Any missed appointment without prior cancellation "no show/no call" will be charged the cost of the scheduled service and must be paid prior to your next session.
