Insurance & Payment Information
Stephanie Staveland, LCSW is an in-network provider with the following insurance plans:
Premera Blue Cross Blue Shield, Aetna, and Cigna.
If you have a different insurance provider, we are happy to bill out-of-network on your behalf. Please check with your insurance company to understand your out-of-network benefits.
Payment is due at or before the time of your session and can be made using cash, debit/credit card, or an FSA/HSA card. A valid credit card is required to be kept on file, and your estimated copay or session fee will be charged at the time of service.
We are not currently able to accept Medicaid or Medicare.
Cancellation Policy
We understand that unexpected situations arise and you may need to cancel or reschedule an appointment. We kindly ask that you provide at least 24 hours notice for any cancellations. Appointments canceled with less than 24 hours’ notice will incur a $75 late cancellation fee.

No-shows appointments without prior notice will be charged the full cost of the scheduled session, which must be paid before your next appointment can be scheduled.

90791 – Diagnostic Evaluation / Initial Intake Assessment: $350

90832- Individual Psychotherapy, 30 minute: $175 per session

90834 – Individual Psychotherapy, 45 minutes: $200 per session

90837 – Individual Psychotherapy, 55 minutes: $225 per session

90835 – Group Psychotherapy, 90 minutes: $50 per session
